A mother of three who was diagnosed with cancer 14 months after she was initially given the all-clear last night questioned why concrete action wasn't taken four years ago when Prof Rajnish Gupta raised queries over the resourcing and support of services.

Rebecca O'Malley (41) from Ballina, Co Tipperary, who lost vital time in her breast cancer treatment following an error with a Cork University Hospital biopsy sample two years ago said the revelations about services in Barringtons' Hospital represented the "worst nightmare" for the women involved.

Ms O'Malley said that yet another cancer screening investigation raised very serious questions about the overall system. There was an urgent need for an audit of cancer care services and there needed to be an improvement in procedures to ensure that those diagnosed with cancer were treated as early as possible.
