The Garda Síochána Ombudsman Commission has opened an investigation into the circumstances surrounding a fatal road traffic collision in Co Galway yesterday afternoon in which an off-duty member of An Garda Síochána was involved.

A 65 year-old woman died following the collision which happened between a car and a transit van at Lenamore, Milltown, Tuam yesterday at around 3.10pm.

She was identified today as Margaret O'Donoghue from Bandon, Co Cork.

It is understood the deceased was travelling in the passenger seat of a car being driven by an off-duty member of the Garda Síochána.

The accident was referred to the Garda Síochána Ombudsman Commission (GSOC) by the Garda Commissioner under section 102 of the Garda Síochána Act, 2005 and GSOC investigators are at the scene of the crash.

Four people were taken to University College Hospital in Galway following the crash and traffic diversions were put in place.

The Garda Ombudsman Commission confirmed an investigation is underway but said no further details are available at this time.
